Calculus option

I'm aware that calculus quantification is not on the avdc site, however I think most practices utilize this anyway. I've always seen it described as: CI, CII, CIII for calculus mild, moderate, and heavy and we would love to see this option added to the dental chart.

    We also use tartar/calculus scoring on every tooth - graded 1-3 (os c/s, c/m or c/h), but I believe it would be more accurate to use the scoring system indicated at http://www.vohc.org/Scoring_Dublin_2016.pdf.
