Not all teeth fit into these three categories. If extraction is planned for a seperate procedure the tooth is not healthy or extracted. Perhaps a root canal is planned? It is miss-leading to put the abnormalities under a 'healthy' tooth.
Perhaps its the product of using the multi-select button to add comment to multiple teeth at once from the front screen. Thanks for the tips I'll do that.

Thanks for submitting this idea. If a tooth is neither healthy, missing nor extracted you can just leave it in the standard state and do not apply one of these three states. By tapping on the individual tooth itself you can add notes and under the Treatment section, we even have a X Recom. option. Please see my screenshots attached.
If you have additional thoughts I'd love to hear them but basically don't mark a tooth as healthy if it is not healthy :)
