Last Name on Preanesthetic Sheet to Use to Log Controlled Drugs

The preanesthetic sheet does not include patient's last name. This would be extremely useful when creating the sheet, drawing up meds, and logging controlled drugs. To log controlled drugs you need patient's first and last name. Right now you have to close the preanesthetic sheet and go into treatment sheet to find the owner's last name.
