Have commonly used pain scoring sheets available

It would be useful to have commonly used pain scoring methods e.g. glasgow or grimace available, in any format but potentially as pre-generated forms. I can generate forms but they are not useful as I am unable to use images, such as those required for grimace pain scoring, within the forms. I pain score each patient at least twice during their routine procedure. It would be helpful to have a digital version rather than wasting paper.
