Data tracking of use of a product code

I was hoping to get data on procedure code, CHANGNB. The data I was in need of is how many times this was used over the last 6 months, and by which users, as well as what notes were entered for this code?

    knowing this data allows for improvement in workflow, lets administrators update product codes if we see the CHANGNB cod is being used frequently enough for a specific treatment or test order.
