Not changing old doses when weight is changed

When a patient's weight is changed, due to fluid loss or something, does not change past doses.


Ex: a 10kg patient received 1 capsule of a 100mg capsule of Gabapentin(10mg/kg) at 8am.

Currently: If by 2pm, their weight is now 9kg, it will retro update the 8am dose to now 90mg Gabapentin to match the 10mg/kg dose. The past now is showing as if a different dose was given, when 100mg capsule was given at 8am.

Preferrably: The old medications (if there was an adjustment due to weight change) is for it to be discontinued, and re-opened with the new doses.
