Medical Records Repeat information

When a note is added to the medical records and then the PDF document downloaded, it will contain the original note and then duplicate entries if changes are made. For example if a Dr. enters SOAP and then changes or adds any entry to the original, there can be multiples of the same SOAP in the PDF document.
