Forms- have the ability to add additional questions based on the previous answer.

Loving using Forms & have been creating a lot of checklists. It would be great if a specific answer prompted which question comes next. For example, if we ask "Vaccination Status" - if the answer is never vaccinated, then the following question would be "would you like this done today". If however, the answer was that that the patient was up to date with vaccines, the next question would ask which ones & when were they done etc. I see this functionality having a lot if use for obtaining patient history also. If an answer was yes, to is the pet vomiting. The following question would be how often, is to related to feeding etc. If the patient is not vomiting then it would skip to the next section.

    This isn't getting enough attention. We could have fewer forms if we could nest questions within previous questions, like most website forms do. For example, surgery consent: what procedure are we performing? Groom or Dental Cleaning. For grooms, a certain type of anesthesia is used, for dental cleaning a totally different kind is used. We'd like go have the consent signed based on which one, rather than having to create two consent forms.
