Create website link for forms

In an effort to go paperless and use the smartflow forms we created admission forms and history questionnaires using smartflow. If these could be accessed via the practice website it would allow clients to fill them out prior to arrival. This would promote smartflow and keep our form type and location consistent.

Smart Flow Forms linked to website Merged

It would be really useful to be able to link smart forms to our website. New clients would be able to fill out the new client form online before they come in and we would eliminate using multiple different programs.
