Ability to have BLS/DNR/ALS not be selected.

In some cases the wishes of the client for life saving measures is not known. Currently there is no way to have this field to not be selected and defaults to BLS. Even though it may be in the patients best interest it has the potential to be against the clients wishes and a possible area of liability for the business.
