Allow bulk update for doctor on duty

Rather than individually altering the doctor on duty for each patient (in order to be linked to billing), have a function to change doctor on duty for all patients in hospital.

Or change for all patients on current whiteboard view.

Manual changes of individual cases can still be made if needed.

Update primary doctor on multiple cases at one Merged

When multiple cases are being transferred to a new primary doctor, it would be a great time save to update all of the cases with a new primary doctor at the same time - versus editing the info on each individual case.
