Put the Anaesthesia notes into the same PDF as the Anaesthesia sheet

Why? At the moment we only see the first few letters of a note on the Anaesthetic sheet PDF so we have to review a second PDF – the Anaesthetic Records PDF – in order to correlate this information and find out what the note says.
