Emailing Check in and Anesthesia release forms to owners to allow minimal contact (COVID) Merged

To allow minimal owner contact and use of the amazing forms created in SF, create the possibility to email the forms to owners with digital signature options to aid in curbside check in, etc during COVID and even after.
