Ability to create & add own 'Categories'

This is on behalf of Metro Animal Emergency Clinic - they would like to be able to add their own Categories on the Treatment Sheet. They like that it's separated into Monitoring, Activity, Fluids, Medications, Procedures, but feel like there should be a Diagnostics category as they don't like putting everything under Procedures.
