Notes added to specific Monitoring, Activity or medicatio, show up in notes but do not indicate what they are about Merged

I have noticed that when you add a note/additional information to a specific item in the flow sheet it adds to notes. This is great but it doesn't include what it was added to. For example, we have a monitoring item Demeanour with drop down options to choose from. One of the drop down options if chosen asks for you to specify signs observed, i.e. rolling, pawing etc. When this appears in the notes section it adds just what is chosen in the drop down box and your additional text it does not indicate that it was noted against demeanour. Another example is if you give a modified dose of a medication, i.e. Prescribed: Butorphanol 2ml IM, only 1ml given so write this amount in the box. When this appears in the notes it just appears as 1ml.
If the notes are used after they are attached to the patient file in the EMS, it could get a bit annoying trying to figure out what 1ml meant.

    This is a feature we're finding big issues with. To fix the problem on the EMS is a real headache and really negates the advantages of smartflow. If it is not possible to fix, it may cause us to revert to paper charts.
