Lock template so that it cannot be changed during the patient's stay (to prevent loss of information)

Currently if a template is changed part of the way through a patient's stay, any treatments that have been completed prior to the change will be removed. When you attempt to change a template after information has been entered for treatments, you will receive a pop-up that gives a warning about loss of information.
We received a request from a clinic for the ability to lock the template once treatments begin, as they are concerned the pop-up may not be enough to prevent someone from changing the template and therefore deleting treatments that had been completed. This would be particularly useful in teaching hospitals where a student may wish to change a template, but they would first need to check with the clinician to do so if it was "locked".
