Address fields (email, cell phone, etc)

Ability to have address fields within Smart Flow and sent from EMR for both forms and S8 (scheduled drug) requirements.
 address, email and cell phone don't transfer back into Ezyvet

    Also, can we please link the "problem list" to a connected form property? Then we can autopopulate reason for visit with the consent form easily.
