change problem list from saved drop downs to free write only Merged

When you add problems to a patient it offers endless autofill options based on previous entries. This causes smartflow to delay in letting you type your new problem in the box or accidentally allowing it to autofill with the wrong problem. It makes this more tedious then necessary.
