Remove page 1 of the Anaesthetic PDF Merged

Why? We have the information already in our Pre-Anaesthesia and Sedation Record form. It adds a page to the Anaesthesia PDF.
Ivan mentioned that this was included at the request of Guelph Vet School and he also mentioned it would be easy to remove. Could it be made into an Anaesthetic setting to include/not include?
